Educational Objectives

After completion of this activity, pharmacists will be able to:

• Identify individualized glycemic targets and patient factors that may indicate immediate intervention in people living with T2D requiring insulin therapy

• Determine targeted recommendations for insulin modification that align with best practice guidelines based on glycemic data and current regimens

• Apply principles of shared decision-making to resolve specific patient-related barriers to optimal insulin therapy use
